If, since the clinical system merge between Laurel Bank Surgery and the Village Surgeries Group, you have lost access to the NHS App or Patient Access, and are unable to relink your own account, or relink a proxy account, please complete this form.

Unfortunately, it appears that this system transfer has completely removed some online services accounts. We have raised this with our systems supplier.

NOTE: We strongly recommend all patients switch from Patient Acccess to the NHS App.
